We require a valid credit card in the patient name and a $50 deposit for all consults and service appointments before booking which applied towards the services performed. The patient is responsibility to call is if they cancel or reschedule for deposit if they comply to policy because the system won’t automatically transfer the deposit made and a duplicate deposit maybe taken. We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting a much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit due to a seemingly ‘’full” appointment book. The Botox Bar requires that a credit card number be kept on file to reserve your scheduled service. If an appointment is not cancelled at least 24 hours in advance, you will be charged a $50 fee.
SCHEDULED APPOINTMENTS
We understand that delays can happen. However, we must try to keep the other patients and medical staff members on time. If a patient is more more than 10 minutes past their scheduled time, we have the right to reschedule the appointment. If you are more than 15 minutes late for an appointment, The Botox Bar will charge a $50 fee.
All patients that have not been seen at the practice in the current year for any type of medical spa services that are offered at our facility must have a yearly intake done with a midlevel provider or higher by law according to Texas Medical Board (§193.17). This is done yearly to be able to have any medical procedure done in our office. Patient safety and compliance with Texas medical board is number one with our practice. There is a $50 fee for the yearly medical clearance which is done by our PA-C via phone or during the visit.
